Forms & Documents

There are five forms you need to register for our school year Extended Day Enrichment Program and they are all contained in the Registration Packet below.
 
Before you turn in a packet, please verify with our office that there is space for your child at your requested site. If there is space, please print and fill out a separate copy of the forms for each child you are enrolling. Be sure to read our policies in the Parent Handbook (below).

 

Please turn your registration packet(s) in with a check or money order (NO CASH or cards) payable to COUSD for the registration fee. We kindly ask that you include your child’s name in the memo so we can correctly apply the payment to your account.

You can turn the packet in with your registration fee at our office: Room A-1 on the Sunflower Campus (1505 S Sunflower, Glendora, CA 91740). Look for the colorful picket fence on a gate. We are the first door beyond that gate.
